Sample Notice of Intent Form for Colorado Homeschoolers


One of the main requirements of the Colorado homeschool law (CRS 22-33-104 and 22-33-104.5) is to compose and file a Notice of Intent once a year. There's no 'actual form' to fill out - you just compose the notice on your computer or a piece of paper - but there are specific pieces of information you're legally required to include: your child's first and last name, age (not grade level), home address, the date you will begin (at least 14 days in the future) the number of hours your child will be in attendance, where you will submit the standardized test or evauation results (only applies if this happens to be a testing or evaluation year), and last but not least, today's date and the signature of the primary homeschooling teachers. Which is usually you and your spouse, but could also be grandma or grandpa, Aunt Isabel, Cousin Ferris Bueller, or the child's legal guardian. If you need further clarification, please refer to the actual Colorado Statute or my simple explanation of what all the legal mumbo jumbo means.

First, I'll provide a sample draft, showing you what information you need to fill in. Beneath that, you'll see an actual Notice that you can use. Download it, print, fill out, mail. Or copy and paste the text into your word processing program, change the font, etc. Whatever works best for you!



Sample Draft

Dear Sir/Madam,

This Notice of Intent is to notify you that I will begin home educating my child (full name) _________________________ 14 days from today, beginning on (date) ____________.

My child is (age) ________ and resides at (primary home address) __________________________________________.

The number of attendance hours this school year will be ______ (688 hours is the minimum. You can also write it as '172 days, 4 hours per day.' Both mean the same thing.)

(OPTIONAL) Test or evalution results will be submitted to (name of independent private school or school district) _________________ upon completion of my child's 3rd, 5th, 7th, 9th or 11th grade. )
